This is the gold standard for the detection and quantification of GERD. At the University of Washington, we, as a matter of routine, simultaneously evaluate both the proximal and distal esophageal acid exposure. Normal pH monitoring should prompt a thorough work-up to rule out other etiologies, because these patients have an inferior result with surgical therapy. This test can also be used to correlate reflux episodes with symptom events, often serving as a confirmation of the clinical association. Finally, preoperative pH monitoring serves as a baseline by which to compare studies should the patient have recurrent or persistent symptoms after an anti-reflux procedure.1
