The CXR shows an obvious left pneumothorax. However, the right lung also shows cystic changes in the upper lobe. Following left chest tube insertion, the follow-up CXR (Fig. 86.2) also shows similar changes in the left upper lobe. Subsequent bron-choalveolar lavage showed Pneumocystis carinii. Upper lobe cystic form of Pneumocystis carinii typically appears in those with HIV who are on prophylactic nebulized pentamidine.
