Social history

This is always important. The aetiological role of smoking in coronary and other vascular disease necessitates a detailed smoking history. Consumption of alcohol is also relevant. Although possibly 'cardioprotective' in moderation, excessive alcohol consumption is associated with atrial fibrillation, hypertension and cardiomyopathy (Fig. 1.9, p. II). Enquiry should also be made into caffeine consumption which, when excessive, can cause palpitation. Some recreational drugs can occasionally be associated with cardiac symptoms (e.g. cocaine and chcst pain).
