In courses A and B, 10% of the MTX dose of 500 mg/m2 (= 50 mg/m2) is administered IV over 30 minutes and
90% (= 450 mg/m2) as a 23.5-hour continuous IV infusion. Leucovorin 15 mg/m2 is administered as a bolus at 48, 51, and 54 hours.
In courses AA and BB, the MTX dose is increased to 5 g/m2. The infusion schedule and leucovorin rescue are the same as in protocol for non-B-cell lymphoma (see Table 16-6 footnote).
cMay be replaced with vinblastine 6 mg/m2 if vindesine is not commercially available.
